• You MUST submit this application regardless even if you have completed one in the past.
• If you have already created an account, log in with your previous user name and password and then select Create Application.
• After you have completed your application, watch your email to see if you need to be fingerprinted.
  • If you do, you will receive an Authorization email the following business day.
• If you receive the authorization form email, you need to bring the form to one of the authorized fingerprinting locations. Access a list of authorized locations.
• You will NOT be asked to pay anything out of pocket, if you are asked to, you are not at an authorized location.
